5. Amendment text showing the new wording of the headquarters article of the articles of association

6. Notarized minutes of the General Assembly meeting at which the change of headquarters was approved

   (To be affixed to the General Assembly Meeting Minutes Book pursuant to Turkish Commercial Code Articles 64/4 and 375/f) (1 original, 1 photocopy)

8. Letter of appointment of the Ministry representative

9. Invitation to the general assembly

   (Newspaper announcement, registered mail with return receipt, list of hand deliveries)

10. Board of Directors Agenda Resolution

11. Two photographs of each Board Member
12. If the transfer general assembly includes an election of the board of directors;
    • Board of Directors Division of Duties and Representation Resolution, concerning the division of duties of the board of directors and the manner in which the cooperative is to be represented and bound. This Resolution will be certified by the Trade Registry Directorate. (1 original)
    • Specimen signature/signature declaration of the Board members authorized to represent and bind the cooperative (If no signature record is held in the databases of public institutions and organizations, or the record cannot be obtained, the signatures of the persons concerned are submitted to the directorate either by having them physically notarized or by making a written declaration at any Trade Registry Directorate.)
